(1) Jugular venous pressure
Jugular venous pressure의 상승
prominent X descent : atrial relaxation
abscent diastolic Y descent : atrial emptying
(2) EKG
electrical alterance(가장 특징적)
low voltage of QRS
nonspecific ST-T change
(3) Chest X-ray
심장이 구형 또는 물주머니(water bottle) 모양
pulmonary congestion은 드물다
(4) Echocardiography
가장 도움이 되는 검사
large pericardial effusion
right atrium collapse, diastole시 right ventricle collapse
(5) Catheterization
`equalization of pressure` : 상승되어 같아짐
RA = DRV = DPA = PCWP = LA = DLV(D=확장기말)
6) Treatment
Cardiac tamponade 치료의 기본은 심낭강의 배액(pericardial drainage)
수축기 혈압 낮을때에는 수액투여
이뇨제는 사용하지 않음
(1) Pericardiocentesis(심막천자)
일시적인 방법으로 심장초음파검사하에 시행하는 것이 안전
심낭삼출액이 반복되어 발생하면 반드시 외과적 배액법을 시행
(2) Surgical drainage methods(외과적 배액법)
A) Subxiphoid pericardial window(검상하 심막창)
국소 또는 전신만취하에서 시행
xiphoid process하에서 횡경막을 박리하고 심막에 접근
가능한한 크게 심막을 절제하여 심막창을 만든 뒤 배액튜브를 거치시킴
